Community Health Workers are educating our surrounding community, cooperating with us in outreach campaigns and free health services.

Extended Annual Check Up
Add these extended services to your FREE annual check up consultation:
- Ultrasound
- Blood Glucose Test
- HIV test
- ECG
- Skin Cancer Screening
- Clinical Breast Exam / Prostate Exam
Fee: TZS 30,000/ month
Laboratory Test Discount
Do you require regular laboratory tests to monitor your health or do you just want to be on the safe side? Add this discount and receive a 15% price reduction anytime you need to come to the clinic for laboratory tests during your membership.
Fee: TZS 15,000/month
Spouse Add-On
Do you have a partner who you would like to benefit as well? You can easily add him/her to your plan and save 30% on their basic myHealth Plan and all their add-ons!
Child Add-On
Your children (<18 years) are able to benefit from your subscription as well! Add as many as you like to your account at only 50% cost for their basic myHealth Plan and all relevant add-ons.
Community Outreach Support
Do you want to have a bigger impact? Through your membership with Urban Care myHealth you are already supporting 1 Community Health Worker every month. With this add-on you can choose to extend your support further on our outreach activities. You will receive a monthly bulletin that informs you on our initiatives that were made possible through members such as you.
Fee: TZS 100,000/month
Patient Treatment Support
Do you want to expand your impact even further? Through your membership with Urban Care myHealth you are already supporting 1 Community Health Worker every month. Treatment of patients with lower financial means is subsidized through various channels, and now you can directly contribute to their vital treatment. With this add-on you can choose to extend your reach. You will be supporting the treatment of up to 5 patients per month (depending on the kind of treatment). You will receive a bi-monthly bulletin that shares success stories among our patients that have been made possible with your contributions.
Fee: TZS 250,000/month
Terms & Conditions for Urban Care’s myHealth
These terms and conditions (“Terms”) govern your participation in the Urban Care myHealth Plan (“Membership Plan”) offered by Urban Care Company Limited (“Provider”). By enrolling in the Membership Plan, you agree to comply with these Terms.
- Membership Overview
- The Membership Plan provides access to specific health services and benefits as outlined in the plan details provided by the Provider.
- Membership is valid for a specified period as indicated in the plan details.
- Membership Eligibility
- Individuals eligible to enroll in the Membership Plan must meet the criteria set forth by the Provider.
- Membership is not transferable and is valid only for the enrolled individual.
- Membership Fee
- The Membership Plan requires payment of a membership fee, which is non-refundable unless otherwise specified by the Provider.
- Services Covered
- The Membership Plan covers specific health services and benefits as outlined in the plan details.
- Covered services may include but are not limited to consultations, diagnostic tests, preventive care, and wellness programs.
- Non-Combination with Existing Health Insurance
- The Membership Plan cannot be used in conjunction with existing health insurance policies that cover the same services included in the Membership Plan.
- If you have existing health insurance, you acknowledge that the Membership Plan benefits are separate and cannot be claimed or reimbursed through your insurance provider for services covered under the Membership Plan.
- Exclusions
- The Membership Plan does not cover services not explicitly outlined in the plan details.
- Costs associated with services received outside of the Membership Plan benefits are the responsibility of the member.
- Membership Renewal
- Membership renewal is subject to the Provider’s policies and terms applicable at the time of renewal.
- The Provider reserves the right to modify or discontinue the Membership Plan at any time.
- Termination
- The Provider reserves the right to terminate a membership for failure to comply with these Terms or for any other reason deemed appropriate by the Provider.
- Privacy
- By enrolling in the Membership Plan, you consent to the Provider’s use of your personal information in accordance with applicable privacy laws and the Provider’s privacy policy.
- Limitation of Liability
- To the extent permitted by law, the Provider shall not be liable for any direct, indirect, incidental, consequential, or punitive damages arising out of or related to your membership in the Membership Plan.
- Governing Law
- These Terms shall be governed by and construed in accordance with the laws of Zanzibar.
- Agreement
- By enrolling in the Membership Plan, you acknowledge that you have read, understood, and agreed to these Terms.
